Chemconsult® online goes past the 16,000 mark
The number of Chemconsult® online medication consultations performed by HIP group pharmacies since the introduction of the service last year has gone past the 16,000 mark, and the number of interventions - that is where the consultation picked up an error (for example an incorrect dose or a conflict with another medication) - is around 5,000*.
The most common error to date is ‘incorrect dosage for age/weight’ which has occurred in 28.7% of all consultations to date.
What is Chemconsult®?
Chemconsult® is a proven 10 point system for medication consulting by a pharmacist, which ensures consistency in advice delivery to the patient and thereby has the potential to reduce medication problems, improve medication compliance and track medication performance in terms of supply and potential adverse drug reactions or other misadventure.
*4731 interventions - 28.7% of all consultations recorded - have flagged incorrect dose for patient's age and weight. Potential drug interactions have been flagged in 972 (5.9%) of all consultations
